Deductibles and Co-Payments are due at the time of your visit and may be paid by check or debit/credit card. Any amounts not paid by your insurance carrier are due as soon as you receive an Explanation of Benefits from them. Our billing staff will gladly assist you with copies of reports and any other information requested by your primary and secondary insurance carrier(s) as well as with any questions you may have about your bill. If you are experiencing financial distress, please contact the practice office to arrange a payment plan.
September 20, 2016 Update:
We are currently contracted as "In-Network" for PPO and POS/indemnity (but not HMO) plans with:
- Blue Cross
- Blue Shield
- Medicare (TRICARE, etc.)
If your insurance company is not listed, please call our office so that we can contact them to confirm your benefits for unaffiliated providers.
Please note that we are not contracted with Medi-Cal (called "Gold Coast" in Ventura County) and that we do not currently accept "Worker's Compensation" or "Vehicle Accident" assignments of benefits.
